Epilepsy Foundation Multicultural Outreach Program Announces Funds For Social Determinants Of Health Mini Grants

The Epilepsy Foundation Multicultural Outreach Program announces the availability of funds to award up to 10 Social Determinants of Health (SDOH) mini grants during fiscal year 2023. These grants help to address determinants of health that impact people living with epilepsy in rural areas.

Grant funds of $10,000 per applicant will be awarded to Epilepsy Foundation affiliates and epilepsy-related, community-based organizations that are in collaboration with multi-sector agencies, such as rural health clinics, community health workers (CHWs), or areas of health education centers.

This is a limited competition application process that is funded through the cooperative agreement between the Epilepsy Foundation and the Centers for Disease Control and Prevention (CDC).

Multi-sector organizations are eligible to apply but must be affiliated with an Epilepsy Foundation affiliate, epilepsy-related agency, or regional staff.

Grant Purpose

Applicants will be required to propose training, marketing, awareness, linkage/referral and/or outreach activities with multi-sector organizations to achieve the following:

  • Increase awareness of epilepsy-disparities among diverse populations
  • Increase epilepsy knowledge among social service and public health/health education providers
  • Increase local linkage and referral systems to identify more people with epilepsy from diverse populations
  • Reduce unmet social needs of people with epilepsy related to rural areas, such as transportation, access to specialty care, or broadband access

Letters of Interest Due February 1, 2023

The Letter of Interest (LOI) is due no later than Wednesday, February 1, 2023. Please send the completed LOI to Multicultural Outreach Director Thometta Cozart MS, MPH, CHES, CPH by email to tcozart@efa.org. Please also email Thometta with any questions you may have before the deadline.
