Transportation can be a significant challenge for people living with epilepsy and seizures. Depending on their seizures and where they live, some people may be unable or restricted from driving.
Results from a survey conducted by Kantar Health on behalf of SK Life Science, Inc., reveal that for people with epilepsy, transportation can be a significant challenge. Some people may even be restricted from driving, depending on the severity of their seizures and where they live.
Through the Epilepsy Foundation's partnership with SK Life Science, Inc., eligible individuals with epilepsy or seizures may receive one (1) Uber rideshare transportation voucher every twelve (12) months, while funding is available.
Depending on your eligibility, you may receive:
- $300 Transportation Voucher - Available to eligible individuals who are unemployed and need assistance with transportation to medical appointments or services; or
- $500 Transportation Voucher Available to eligible individuals who are currently employed and need assistance with transportation to medical appointments or services and/or transportation to and from work.
You may receive only one voucher every twelve (12) months.
Who Is Eligible?
To qualify, you must:
- Be at least 18 years old.
- Be a current resident of and live within the United States or Puerto Rico.
- Have a documented diagnosis of epilepsy or a seizure condition.
- Meet the program's transportation eligibility requirements.
- Have a household income at or below 400% of the current Federal Poverty Guidelines issued by the U.S. Department of Health and Human Services.
- Provide the required proof of income and other supporting documents.
- Have a valid email address and phone number.
- Have an active Uber account, or be able to create one. You must also have a valid alternative payment method available in case the cost of a ride exceeds the amount covered by your voucher.
- Meet the additional requirements for the type of voucher you are requesting.
Proof of Income
You must provide your most recent federal tax return as proof of income. Income eligibility is based on your household size, which includes the total number of adults and children living in your household at the same address.
If you are married, live with your spouse, and file separate federal tax returns, you must provide both your and your spouse's most recent federal tax returns. Your combined Adjusted Gross Income (AGI) will be used to determine your eligibility.
If you do not file a federal tax return, you may be asked to provide alternative documentation of your income or financial status. All income documentation is subject to verification.
Acceptable documentation may include:
- Notarized Affidavit of No Income: A sworn statement confirming that you received no income during the period specified dated within the last 30 days.
- Public Benefit Documentation: An official letter or statement from a government agency showing the benefits or assistance you receive dated within the last 30 days.
- IRS Tax Transcript: An official IRS record of income or tax information dated within the last 6 months.
- Employment Verification or Offer Letter: A signed document from an employer verifying your employment and income, dated within the last 90 days.
- Pay Stubs: Your two (2) most recent pay stubs.
- Statement from a CPA or Trust Manager: An official statement verifying your income or financial status, dated within the last 30 days.
- Bank Statements: Statements covering the most recent three (3) months.
- College Financial Aid Documents: Official financial aid documents that provide information about your income or financial eligibility. dated within the last year.
- Notarized Guarantor Affidavit: If applicable, a notarized statement from a guarantor regarding your financial situation, dated within the last 30 days.
Physician Documentation
You must provide a verifiable letter from your current physician treating your epilepsy or seizure disorder. The letter must:
- Be addressed to the Epilepsy Foundation of America.
- Be on the physician's official letterhead.
- Be signed by the physician.
- Confirm that you have a diagnosis of epilepsy or a seizure condition and that you are unable to operate a motor vehicle.
- Be dated within 30 days of your rideshare application.
Additional Requirement for a $500 Transportation Voucher
If you are applying for a $500 Transportation Voucher, you must also provide your two (2) most recent pay stubs.
Name on Documents
The name on your application must match the name shown on your supporting documents. If the names do not match, you may be asked to provide documentation explaining the difference.
Terms and Conditions
- Eligible individuals may receive only one (1) voucher every twelve (12) months, while funding is available.
Applications must be completed and submitted by the individual who will receive and redeem the voucher.
If a disability, cognitive impairment, or other documented medical condition prevents an individual from completing or submitting the application independently, a legally authorized representative or another person authorized by the individual may assist with or complete the application, as applicable.
The program may request documentation from a qualified healthcare provider confirming the need for assistance. The documentation does not need to disclose a specific diagnosis unless that information is necessary to determine program eligibility.
Nothing in this requirement is intended to limit an individual's right to request a reasonable accommodation when applying for or participating in the program.
- Vouchers are non-transferable and may only be used by the individual to whom they are issued.
- Vouchers may be used for eligible transportation services only.
- Vouchers expire six (6) months from the date they are issued.
- Any unused voucher balance remaining after the expiration date will be forfeited. Unused funds cannot be transferred to a future voucher or credited to your Uber account by Uber or the Epilepsy Foundation.
- You are responsible for any fees or ride costs that exceed the amount covered by your voucher.
How to Apply
- Review and agree to the Terms and Conditions. By proceeding with your application, you confirm that you understand and agree to the program requirements.
- Call the Epilepsy & Seizures Helpline at 1-800-332-1000, Monday through Friday, from 8:00 AM to 3:00 PM ET, excluding holidays. An Information Specialist will review the program requirements with you and conduct an initial eligibility screening.
Submit your requested supporting documents by email to [email protected] or by fax to 301-731-3868.
All requested supporting documents must be received within ten (10) calendar days of your initial contact with the Helpline. If the required documents are not received within this timeframe, your application may be closed, and you may be required to reapply.
- Do not send supporting documents until an Information Specialist has asked you to submit them. Documents received without an associated application or request may be deleted and will not be considered.
- After all required information and documentation have been received, an Information Specialist will notify you of the status of your application within ten (10) business days.
